Release Statement and Waiver of Liability:
By submitting this form, you agree to and follow the following terms and conditions:

In consideration of the Birthday Party/Event provided by the Happy Hands Art & Pottery LLC facilities, in which the party/event is held, I do hereby allow my child/children listed above to participate in the party/event and do hereby release, waive, discharge, and covenant not to sue Happy Hands Art & Pottery LLC, its owners, instructors, and employees from from any and all liabilities in case of any incident or accident occurring from my child’s participation in the art activities.

Medical Release:
I understand there are no medical facilities at The Happy Hands Art & Pottery LLC, and that Happy Hands Art & Pottery LLC will not be held responsible for any medical care. I give permission to Happy Hands Art & Pottery LLC to seek emergency medical treatment on behalf of my child in the event of any health emergencies. Happy Hands Art & Pottery staff is trained to call 911, then reach parents/guardian and/or emergency contacts.

I accept the responsibility to furnish any allergy information for minor participating students or party guests to Happy Hands Art & Pottery LLC in the event this information might be needed to be made available to emergency medical services. I further authorize Happy Hands Art & Pottery LLC to retain the services of emergency medical persons to treat the child, and I accept full financial responsibility for any charges arising from said treatment.

Viruses/Illnesses Warning & Disclaimer:
Anyone who is feeling ill, please stay home. Despite our careful attention to maintaining a cleaning facility, Happy Hands Art & Pottery LLC cannot guarantee that students, visitors, party guests will not become exposed to or infected with transmittable illnesses. By using our facilities or participating in any of our programs, participants are acknowledging and accepting these risks.

Release of Liability:
All guests, parents and legal guardians of students and party guests, all fully waive and relinquish all claims against Happy Hands Art & Pottery,LLC ; it’s owners, instructors and employees from any accidents, injuries, loss, or damage which may be sustained by any guest. Happy Hands Art & Pottery , LLC is not responsible for any personal belongings left at the facility during any events or activities.
Happy Hands Art & Pottery reserves the right to use photos or videos taken at our programs and events on our website, blog, social media or in printed materials.

By signing this release form, using our facilities, and/or participating in on-premises Happy Hands Art & Pottery, LLC activities, you are acknowledging and accepting all the Terms and Policies stated above.
